I would recommend "a modern method for guitar" volume 1.

It comes with a dvd with 14 hours of music reading instruction. However, i personally dont think it coveres rhythm notation in depth. Great book though

Berklee Press has another great book for site reading. It is called READING STUDIES FOR GUITAR.
